Summary:
Allows new parents to refuse to disclose their personally identifiable health information on the facility worksheet for birth certificate. This bill also repeals the requirement that the birth worksheet be retained permanently; the House changed that requirement to seven years. The Senate amended the bill to remove identified information (such as birthplace and social security number) from the facility worksheet.
